The idea of an episiotomy or perineal tear during labour can be frightening for many mums-to-be. But it doesn't have to be! With the right knowledge and targeted preparation, you can do a lot to minimise the risk and have a relaxed birth. Here you can find out what you need to know about the perineum, the labour process and the best prophylactic measures. Where is the perineum and what happens at birth?

The perineum is the area between the entrance to your vagina and the anus. During labour, this area is stretched considerably as the baby moves through the birth canal and finally sees the light of day. This is an intense but also incredible experience - your body is made for it! With the right preparation, you can optimally prepare your perineum for this task.

Types of perineal tears and when an episiotomy is necessary

There are different types of perineal tears, which are categorised according to their depth. We use the abbreviations DRI to DRIV to categorise the injury. A first-degree perineal tear (DRI) is only an injury to the skin. In the case of a third and fourth degree perineal tear, the sphincter muscles of the anus are also injured and must be sutured. Most tears are superficial and heal quickly. 

An episiotomy, on the other hand, is only performed if it is medically necessary - for example, if your baby needs to be born quickly to avoid complications. In such cases, the incision creates more space and helps your baby to be born more quickly.

The most important thing: both a tear and a cut are sutured after the birth under local anaesthetic and heal in the puerperium. With the right care, you will soon feel well again.

The best prophylaxis: what you can do to protect your perineum

The good news is: there are many ways in which you can optimally prepare your body for the birth. These measures will not only help your perineum, but also your general well-being during pregnancy:

  • Pelvic floor trainingA strong and flexible pelvic floor helps you during labour and supports healing after the birth. In our courses you will learn specific exercises.
  • Exercise during pregnancyRegular, gentle exercise such as yoga or walks promotes blood circulation and keeps your body fit.
  • Perineal massageRegular massage can make the tissue of your perineum softer and more elastic. This is particularly helpful in the last few weeks of pregnancy.
  • Hay flower steam bathIf you are not allergic, hay flowers are good for preparing the tissue for labour. A sitz bath of hay flowers relaxes your pelvic floor and perineum. Steam can be too hot. That's why I recommend you take a sitz bath so that you don't get burnt.
  • Upright birth positionsBirth positions in which you use gravity can reduce the pressure on the perineum. This also applies to water birth, which is also gentle on the tissue.
  • Slow birthInstead of actively pushing your baby out, you should concentrate on moving naturally with the contractions. Through conscious breathing and the right technique, the baby can be born more gently.

This prophylaxis can not only reduce the risk of a perineal tear, but also ensure that any injuries are smaller and less painful.
